[cisco-voip] Extra Digits

I have a new CCM 4.2(3) deployment, the voice gateways are MGCP and I  
am using the service parameters to prefix the 9 or 91 on inbound  
calls to facilitate easy dialing from the Missed/Received Calls  
Directory.

The issue is when a user places a local call by dialing 95551212, the  
calls in placed and once it connects the phone display shows the  
dialed digits as 9195551212.

Where is the extra 91 coming from? It appears to just be cosmetic,  
the CLID is displaying properly to the called party and the Placed  
Calls Directory shows the correct dialed digits. Users are freaking  
out about and it is odd so I would like to fix it.

The DNA tool doesn't indicate anything that would apply the extra  
digits; no translation patterns or prefixed digits in the Route Lists  
or Gateways.

Phones are 7941's and 7971's running 8.2.2sr1
