[cisco-voip] A bug? Random authentication errors when logging into EM. CUCMBE 6.0(1)

Hello. I did a bug scrub but couldn't find anything. I'm ready to open a TAC
case.

Random users complain at least once or twice a week that they cannot login
to their extension mobility profile. They get an error on the phone stating
"authentication error" after they enter their pin and password. The only fix
is to login to the end user config and reset their pin. I'm curious if
anyone else has seen this?
