You did not mention the type of phone nor the phone load being used. There were a couple of similar issues resolved for the phones below in the listed phone loads.

I'm seeing random phones on call manager express go into a state where if you pick up the handset there is no dial tone. You have to also press a line button to get dial tone. If I reset the phone it will go back to normal. Anyone seen this?
