Jason,

Is your outbound carrier and inbound carrier the same?  CLID  
frequently gets dropped between carriers.  Place an outbound call  
that loops back to you, i.e. dial your desk phone number from your  
desk phone as (outside access code)(10 digit number).  does CLID come  
through?
