Nick,
I have seen this problem on cm5 when the dblcnqueue fills up for various
reasons (once had the problem that a cdr receiver was not taking files
and things started to go downward from there...)

Check wether the Change Notifications queued to DB and to Memory
counters in rtmt are returning to zero or not.

If they are not, you can check the content of the queue via cli:
Run sql select * from dblcnqueue

If this is the problem definitely talk to TAC
