[cisco-voip] disconnected calls??

hi 
i got ccme router 2800 and pri line.  CCME 3.3
intermittently my pstn calls are disconnected.
there is no error under E1 controller. it happens randomly but i dun know why. 
do you think it is because of there is no qos in the switch: i created voice vlan and data vlan but did not configure any mls command.
also what is the message        DisconnectCause=1F       when i run the command sh call history voice  id 

phone loads:   7.2(2.0)                7940/7960 
phone app  load ID   P00307020200

ccme upgrade or qos or phone firmware?
