[cisco-voip] CCME "line not available" when trying to use conference feature

I recently upgraded a CCME 3.1 to 4.0(0).

I didn't do a clean install, 'no telephony-service',
but rather just upgraded the IOS and rebooted. 

Users are currently getting a 'line not available'
when trying to use the conference feature on their
phones. Is this a problem that I created by not
erasing the old CCME telephony-service configuration?
