[cisco-voip] for voip connection with an isp which settings

Hi 

i use ccm 4.1.3 in lan. But also i made an aggrement with an isp(i have already have internet conection with the same isp) so that i will use their backbone for international calls.
CCM is behind a FW. Also for pstn calls i have on 2821 gateway . CCM and my gateway has mgcp connection

what should i do on FW and CCM for route my international calls to the isp/ 
How the ip phones will send up packets to the softswitch of isp
Also which ports for ip phones and CCM do i have to open from firewall

design is below

CCM > CISCO2821 gateway(mgcp) > ip phones   

Firewall > internet router with ISP
