[cisco-voip] Weird phone issue

Chris, I would update the firmware on that single phone to the latest code
and test again. (like 8.21 at least)
This can be done by loading the FW into the TFTP server and specifying the
Phone Load Name on the Device config page.

Be very careful to check the Device Defaults page and copy down the current
settings for the model you are updating.
I've seen on many occasions that the defaults will be updated automatically
by the install. I think this is why Cisco offers a zip file of the loads.
