[cisco-voip] CM 4.1(3)SR5

I've seen this almost exact problem  once before.  It was caused by the
carrier forgetting to assigning a PSAP location to the circuit.

What is configured on your route pattern for "911"?

Are you using CER? (Cisco Emergency Responder)?

What number should be passed to the PSAP when dialing 911 if you are not
using CER? most likely the main number to the business.

Does your Route Pattern or Route List Restrict CLID?

If not

Then make sure you check you calling Parties Mask, but change the CLID
to the 10-Digit Main Number, make sure that you allow on your
RouteList(on the Route Groups for the RouteList).
