The first thing that comes to mind is that the extension being passed to
Unity is that of the incoming RP rather than the 1016 extension.  Is VM
getting delivered to the right mailbox?  If Unity 4.0(x), you can use a
custom VM profile in CCM.  I believe later versions of Unity have a "use
redirected number" feature available.
