We ran into a very similar bug. Info below. TAC said it would be fixed
in CM 6.1 and a ES for 6.01. (don't know exact ES)

Don't know about CM 5.1

This is Eric with the Enterprise Voice TAC and I will be working this
service request with you. This issue is very similar and might be the
same as a couple of known bugs for Callmanager 5.x:

CSCsj33132 Insert UDP is Unsubscribing Service from UDP Template

CSCsj33055 bulk insert phone or UDP does not add phone service if no
svc parameters

My apologies for the delay. I reviewed your BAT.txt file and the
template, everything is correct and SHOULD be working, so I think this
bug has been carried over to your 6.0 version. I will be checking with
the developers on this, in the meantime please try the workaround:

For Device Profiles:

1. Using a template

- Add a dummy parameter to the EM service. Save it and click on Update

Subscriptions.

- Subscribe EM service on the UDP template. Ignore if already done.

- Upload the Insert file for specific details transaction.

- Submit a job with the uploaded file and the UDP template on which EM
is

subscribed.

2. Using a CSV file

- Add a dummy parameter to the EM service

- Specify this dummy parameter with blank values in the CSV. Sample file
below

- Upload the Insert file for specific details transaction and submit a
job

Once you have inserted DPs, you can go to IP phone service page, select
the EM

service. Delete the dummy parameter and click update subscriptions.

sample:

************

DEVICE PROFILE NAME,DESCRIPTION,DIRECTORY NUMBER 1,SERVICE NAME
1,SUBSCRIBED

SERVICE NAME 1,SUBSCRIBED SERVICE NAME ASCII 1,PARAMETER NAME
1,PARAMETER VALUE 1

DP-1,DP1,2001,Extension Mobility,Extension Mobility,Extension
Mobility,dummy,

DP-2,DP2,2002,Extension Mobility,Extension Mobility,Extension
Mobility,dummy,

************

For phone devices:

1. create bat phone template and subscribed it to the EM service

2. define an update phones job:

enable extension mobility

log out profile: use current device settings

add services from this teamplate: <select template defined above>

run immediately

Make sure to check the box to the left of the line item in the BAT job

definition. That signifies to BAT that wish to synchronize all selected

phones to the value specified.

This will not prevent the service from being unsubscribed to the
template, but should add UDP's which do contain the EM service.

Subject: [cisco-voip] Issue inserting User device profiles in CCM5.1
with BAT

I came across an issue in CCM 5.1 when importing User device profiles. I
created a template and created a login/logout service and subscribed the
service to the UDP template. I create a file with BAT for the User
Device Profiles. I validated it, and imported the Device profiles and
then the Users. When I go back to the User Device Profiles it does not
have the services subscribed. I have done this more than once, and made
absolutely sure the services is subscribed to the Template, and that I
select the template when importing User Devices profiles. What I also
found is that after each import, it removes the services tat is
subscribed to the template.

Is this a bug in CCM5.1?

I also found that there is no way in CCM 5.1 to update services on User
device profiles with BAT.

Kind Regards
