[cisco-voip] 7911 phones in ccmadmin with CCM Sub IP

Anyone run into this before?

Twice in the last month on different CCM revs and firmware we have had a
situation where several, but not all, 7911 phones are listed in ccmadmin
as having the same IP address as the CCM Sub.  Despite this the phones
are working nominally including all supervisory functions.  The phones
themselves have legit IP addresses on them.  The only function affected
is the Syn-Apps SA-Announce phone paging because it's picking up the
bogus IP address from CallManager.  

CCM has been 4.2(3) and now is 4.2(3)sr3.  The problem has occurred with
firmware 8-0-2sr1 and 8-2-2sr4 (skipped DevPack 36).  It smacks of a SQL
problem but so far I have found nothing to support this nor any errors
in the CCM traces or event logs that seem to relate.
