The problem with using a load server is that you need to apply it to all
phones, and then take it back out.

I've found that taking an outage and temporarily putting the router into
CME mode was much quicker and more reliable, and didn't involve touching
the phone configurations. Once you've reconfigured the router as below,
power cycle the phones by doing a shut/no shut on the switch ports
("interface range" is your friend). When they reboot, they'll DHCP and
option 150 will tell them to connect to the router.

You can also do a:
# term mon
# deb tftp events

If you want to watch the files being served to the phones.

Once all of your phones have downloaded and registered to the router
("sh ephone reg"), we do a reload on the router, then do another shut/no
shut on the phone ports and they register with CM.

Delete existing firmware on the router:
del flash:*.sbn
del flash:*.loads
del flash:cmterm*

Copy the tftp firmware from the publisher:

copy http://10.255.1.10/cmterm_7936.3-3-14-0.bin flash:
! 7914
copy http://10.255.1.10/S00105000300.sbn flash:

For 8.2.2SR4 phone load:

copy http://10.255.1.10/term70.default.loads flash:
copy http://10.255.1.10/term71.default.loads flash:

copy http://10.255.1.10/apps70.8-2-2ES6.sbn flash:

Edit the DHCP pool and change option 150 to be the IP address of the
phone-facing interface:

ip dhcp pool Phones
    network 10.255.104.0 255.255.255.0
    option 150 ip 10.255.104.1
    default-router 10.255.104.1
    lease 7

! Remove the call-manager-fallback command:
no call-manager-fallback

! Configure the router to act as a TFTP server
tftp-server S00105000300.sbn
