[cisco-voip] FXO Goes from Ringing to FXOLS_GUARD_OUT to Ringing and never gets answered by a phone

I have an FXO port that goes from Ringing to Connect to FXOLS_GUARD_OUT then
back to Ringing.  All other ports on the card are working fine and they all
co to the same destination number.  This is an MGCP gateway.  Any thoughts?
