James,
You need to add the prefix (91 I'm guessing?) to the calling number on 
the inbound leg of the call.  Where you do this depends on your 
environment - could be on the gateway directly (H.323), on the gateway 
configuration in callmanager (H.323 or MGCP), via translation pattern, etc..
The downside to this is that the calling number will show the prefix for 
caller ID, as well as in CDR's, etc.
To my knowledge, there's no way to prefix the number so that it only 
appears that way in the missed calls directory - would be cool if you 
could though.
