[cisco-voip] Cisco Agent Desktop 6.4 transfer failed

Dear all,
I have installed CAD 6.4 on agent desktops. The server software version is
UCCX 5.0(2). When agent tries to transfer a call to any number, the caller
hears MOH and stays like that without going through. Both supervised and
blind transfers are not working. What can I do to solve this issue?
