You can set up a translation pattern to add 91 to the calling number and
put it in a calling search space assigned to your gateways. You will
see the 91 added to the calling number in your CDRs too though.

Subject: [cisco-voip] external missed calls
Hi, I am running CM 4.1.3(sr5d) and we need to dial a 9-1-(area
code)(phone#) when making a call to an outside line. My question is
whenever we receive an incoming call from and outside line and the
caller ID picks it up we cannot call it back using the directory on the
phone because it doesn't contain the 9 and the 1. Is there a way for
the phone itself to program the 9-1 in for any external calls being
called back using either the missed or received calls? The placed calls
already have the 9-1 due to that was what was dialed.
