[cisco-voip] Attendant Console call transfer issue

Hi,
Has anyone seen behavior like this before? I am going to get a closer
look at this Tuesday but things we're fine until a recent upgrade from
CCM 4.1(3)Sr4d to 4.1(3)SR5d and the attendant console client was
updated and matches server version. What happens is when the attendant
console user transfers a call (sometimes) it doesn't work and it saids
it is unable to. When the problem happens, the call on the phone and
attendant console display saids "To Conference". They have the same
problem from the phone also when trying to transfer a call. I think it
is calls that get transferred back to the receptionist who handles
most of the calls.
I know it isn't much info for now...
