[cisco-voip] install CCM4.1(3) on HP DL380G5 Server.

I recently installed the OS at a customer site using 2000.4.5a , I also had
tuff time locating the OS install disks for this server but did not have any
firmware issues, I can check firmware details for you when I go back on
site.
