[cisco-voip] Append LD Access code on outbound call

Have telco remove the 3-digit code and configure FAC in CallManager? Or
is telco providing some sort of billing reports?

not likely to be possible. most access codes require inband dtmf. note
this is distinctly separate from the digits send on the isdn d-ch to
establish the call.
only likely way would be a custom IPCCX/IVR, TCL, or CUEA script that:
1. sends the inital setup with called party number
2. waits a period of time, or even better, does tone detection to
determine when the telco prompts for access code
3. sends the access code as inband DTMF
4. connects the calling/called party numbers
Customers i've worked with in the past were not interested in this much
work for a single special user. they usually dedicated 1 pots line to
allow that person outbound calling. remove the LD codes from the pots
line and configure CSS so only the special user can use that pots line.

On the gateway I tried a standard dial peer with "translate-outgoing
called". I tried post-pending a ,022 to the string (, is supposed to
insert a pause and 022 is the telco access code).

I tried "forward-digits extra" which is supposed to send any digits
longer than the length of the destination-pattern. Didn't work.
