[cisco-voip] 15 second delay in Directory Handler when pressing 0

I am having a strange issue in Unity Version 5.x that when someone is in the Directory Handler and presses the 0 for help, the Directory Handler takes 15 to 16 seconds before it transfers to the Operator Extension.  I have created a new Directory Handler with the same results (although the only options are to copy the default or copy an Existing, which happens to be the one I have an issue with).  I have created a new Call Handler to attempt to transfer to on the 0 option.  
 
Anyone have any ideas on what could be causing the 15 second delay?
