[cisco-voip] CER upgrade doc?

Thanks Jonathan, but I still have some questions.

I'm not upgrading CCM.  it is currently 5.1.3 with CER 1.3.2 as this was
what was available at the time of install.  Believe me I really tried to get
CER 2.0 at time of install.

So I'm not doing a CCM upgrade so then how do I migrat to CER 2.0?
