[cisco-voip] AUS Dial Plan error - Error while selecting from Dial Plan table. Reason : 42000

Hi I am trying to install the AUS dial plan on to one of the subscribers of a CUCM 6.1.1-2000 server and getting this error "Error while selecting from Dial Plan table. Reason : 42000".  It installs fine on the Pub and one other Sub but fails on the 3rd server.

Any ideas would be appreciated.
