[cisco-voip] CER ALI submission guidelines

Just want to get some guidance on submitting ALI to a service provider or about the required fields in the ALI like Company ID and Customer ID.  I need to apply for a Company ID from NENA.org, right?  The company ID is the same as customer ID?

ANyone who has experienced dealing with the telco in the LA area?  If yes, which number do I need to call and how do I send them the ALI?
