[cisco-voip] SIP Trunk on Call Manager 4.2

1) The trunk should appear in the gateways/hunt lists dropdown.  It 
would be recommended to put the trunk into a hunt list/hunt group so you 
get a bit more flexibility rather than tying a route pattern directly to 
the trunk

2) you'll need to look at CallManager traces.  Are you actually exposing 
your callmanager to the internet to send calls to your provider??  This 
is definitely a situation where you'd want to set up a router as a 
session border controller/IPIP gateway.  Would also make troubleshooting 
a bit easier without having to dig through CCM traces :)
