I think this is it. In our situation the receptionist is pressing a blf pickup and then her line button to answer a call. I think occasionally she was picking up the handset just before pushing the line. I let her know what we thought was happening and she'll let me know if this is the issue

We are having an intermittent problem with calls coming in and no ability to transfer them.
The softkeys at times available are ONLY: Redial, CFwdALL, or New
Call. (the normal Hold, EndCall, Transfer and More do not appear).
There were no other calls coming in and no one else was on any of the other lines at the time.
Any ideas??
