Ditto (answers within a couple hours)
I always include all the information they would ever ask me. I also
include my CCO ID just to make it easier (probably just a good luck
thing...)

I usually email all licensing issues (even if i don't have an open TAC
case) to licensing at cisco.com and typically I get an answer within an
hour, sometimes less, occasionally more. It appears that as long as
you send it using the email address tied to your CCO account they'll
open a case and tie it back to your CCO ID.
I don't know why, but it seems to work much more quickly/easily than
dialing in or even the using the "case open" tool.

server to register it, but I don't think it will cause you any downtime
if you have the wrong MAC. The licensing guys should be able to update
to your new server without much issue.
