[cisco-voip] mwi not turning on

I have double checked three of the routers in our system and they are
all setup the same way including the one that is not working:
CME:

ephone-dn 143
number 8000....
mwi on
!
!
ephone-dn 144
number 8001....
mwi off
!
Unity Express:

Any other ideas?
