[cisco-voip] FW: Caller ID

I have run into this problem before. Some Telcos do not like to see unknown
as the "calling number" plan. I set the "Called Number" type/plan to
Unknown/Unknown, but I always set the "Calling Number" to National/ISDN.
Haven't had a problem with this setup anywhere.
