Can't answer all your questions, but you're right on MGCP. MGCP gives 
direct control of the gateway's physical interfaces to call Manager, but 
there is no way to control a SIP trunk on the gateway.

Also don't forget to enable
voip
allow-connections h323 to sip
allow-connections sip to h323

for this to work.
