[cisco-voip] CM 5 -> 6 upgrade issues

There were some 7961's that weren't on matching firmware for some reason,
but all my problems were with 7960's and 7914 expansion modules.. and the
firmware was the same before and after the upgrade for both of those. I was
thinking the same thing, maybe some firmware upgrades got hosed up.
