[cisco-voip] PAB in CM6

Last time I looked into this, there was no service parameter.  Which is not
good for DMA upgrades from 4.1.  During one DMA upgrade, I had to edit the
PAB/FD data within the DMA tar file to prepend the outside access code prior
to importing the DMA data into the appliance.
