Per the CUCM 6.x SRND you need 3 commands for protocol-based T38 with
MGCP.
no mgcp fax t38 inhibit
mgcp package-capability fxr-package
mgcp default-package fxr-package

For NSE-based T.38 apparently all you need is the 'no mgcp fax t38
inhibit'. I think you may need to look more closely at what device
(s) this router is faxing to compared to the other routers.

What fax protocol are you trying to use? Try comparing the MGCP
configuration of the problem router versus the ones that work.
