[cisco-voip] Disconnect Cause Code 47 on MGCP Gateway

This is 'resources unavailable'.  Generally, reasons for this are:

-Unexpected Codec mismatch
-No transcoder in the MRGL
-Transcoder has unregistered
-No MTP in the MRGL and 'Fail Call on MTP Allocation failure' is set to true
-DSP errors mid-call
-Lack of available DSPs

Things to check:
Region settings between devices to see if they match, and if not,
check your transcoders.
'show voice dsp group all' at the bottom tells you if you have DSP
allocation failures
Check MRGLs for MTPs
