Google voice does this pretty cleverly I think.  When each device is 
called by Google, you get a prompt first, like "Press 1 to take this call".

If you don't press 1, either because you don't want the call, or because 
it was answered by VM or an answering machine, then the caller leg isn't 
connected.  They just keep hearing "ringing" - I'm pretty sure that 
Google Voice is just playing inband ringing at that time so that the 
caller doesn't know that GVoice is still trying to find you.

If GVoice doesn't get a "1" on any of the outbound call legs that it's 
searching for you on, then it finally dumps the caller to GVoice 
voicemail.  Presto - one single voicemail box.
