Brand new building, just wired up. Punched down from the demark into the
data center, then RJ11 from those ports into the FXO port on the
gateway. Using an analog phone on the ports in the data center works
perfectly.

It might be the Telco or it could be the in house wiring in your
telephony closet
Was your building just recently wired for the fxo? And if so who did
the wiring because I would check there first
Telco's will bill you if they don't find any issues on their stuff
before the demarc

So basically it's a telco issue with the POTS line, and not
something I can fix myself?

OK,
When you are using the fxo port as a two way analog trunk connected to
the Central office/Telco,
This is when a flipped pair somewhere along the Telco facilities can
cause problems
your log tells me that the fxo isn't getting correct voltage wink
because pair has been flipped or rolled or crossed

Also I have provided your log from your last post and underlined and
bolded to indicate what the log is showing me

Still not making a lot of sense I'm afraid. When I've got an
analog phone hooked directly up, it works perfectly both inbound and
outbound. When I've got it connected with a RJ11 to the FXO port on a
3845 (set for MGCP), inbound works, but not outbound. I tried just
switching the RJ45 ends, but that didn't make any difference.

If you reverse the wires that are connected to an fxo it doesn't get
correct voltage reversal and the line that is will drop. This is called
a wink start trunk. So you flip the pair and test your fxo
