There are instructions in the notes for CSCsl48851. Here they are for reference:

For all Unity systems, the ldifde command line is:
ldifde.exe -i -k -c "<ConfigurationContainerDN>"  "<Configuration
container distinguished name>" -s "<Domain controller>" -f " <Path to
Unity DVD 1 disk image>\
Schema\LdifScripts\avdirmonex2k.ldf" -j "<path to Unity log file>"

If you are using the Cisco Unity Bridge, also run:
ldifde.exe -i -k -c "<ConfigurationContainerDN>" "<Configuration
container distinguished name>" -s "<Domain controller>" -f " <Path to
Unity DVD 1 disk image>\
Schema\LdifScripts\omnigateway.ldf" -j "<path to Bridge log file>"

If Unity system is using VPIM also run:
ldifde.exe -i -k -c "<ConfigurationContainerDN>"  "<Configuration
container distinguished name>" -s "<Domain controller>" -f " <Path to
Unity DVD 1 disk image>\
Schema\LdifScripts\vpimgateway.ldf" -j "<path to VPIM log file>"

where:

<ConfigurationContainerDN> is a literal value
<Configuration  container distinguished name> is the distinguished
name for the domain in which you want to extend the schema. For
example, for a domain named voice.cisco.com, you would  use
"CN=Configuration,DC=voice,DC=cisco,DC=com"
<Domain  controller> is the DC on which you want to run the  command
<path to  Unity/Bridge/VPIM log file> is the location where you want
log files to be  saved

So for the domain voice.cisco.com, with a DC of hostname
'dc.voice.cisco.com', and a Unity install disc in drive E:, that would
be:

ldifde.exe -i -k -c "<ConfigurationContainerDN>"
"CN=Configuration,DC=voice,DC=cisco,DC=com" -s "dc.voice.cisco.com" -f
"E:\Schema\LdifScripts\avdirmonex2k.ldf" -j "."
