[cisco-voip] 7940 Phone Unprovided Error

Hi Everyone,

I'm running a Cisco 7940 endpoint with SIP firmware of P0S3-07-4-00, and I
keep getting 'Phone Unprovided'.  It appears the phone is taking some of the
configuration because I am seeing the provisioned extension number in the
top right hand corner.  After going through the phone and looking at the
Status messages it says 'W320 (5) Errors Parsing SIP<mac>.cnf

Any troubleshooting suggestions would be GREATLY appreciated.

Here's a configuration snippet.

*Note: DN1 is a Cisco 7961 phone that has registered without issue.
