Never tried it, but look under my user setting in CUCM7 (don't know when it was implemented) and there is a "Name Dialing" property.

OpenSER with all users set up by alphanumeric usernames.

Just say for the purpose of this discussion that this system at the 
remote end of the trunk cannot be modified.  It (only) accepts calls 
addressed to username at example.com as a SIP URI.

 From my research and the discussion so far we can send to @example.com 
(using SIP Route Pattern by domain, setting up a SIP trunk, etc.)

The sticky part is being able to send to the "username" part of the URI.

If the remote end were configured with a numeric dialplan it could be 
1234 at example.com and I believe it is simple to do that in CUCM.
