you'd look at detailed CCM traces as far as logs go. you could also
lok at the application event log to see if there are transient
connection attempts from taht device.

short of that, you have netmon at your disposal since you're on a
windows based platform. YOu could easily use that and do a packet
capture to see if you're getting so much as a TCP SYN from the remote
phone. you wont see that level of detail in the event log or the CCM
traces.

a packet capture at the remote phone is probably called for as well,
especially if you don't see anything in the packet capture at the
CUCM. YOu're also goign to want to look at the syslog on your
firewalls.
