[cisco-voip] 911 operator gets billing number instead of DID

it's pretty easy once you get the concepts.

each switch port is put in a ERL.  That ERL is assigned a ELIN.  the ELIN is
then updated to your PS ALI DB provider.  then when ever the phone is
plugged into that switch port it uses the correct ELIN that has the correct
location for the PSAP.
