Just to confirm, I want the user to be able to control whether all
calls to him go to the secretary's vm. In other words, the caller wouldn't hear the secretary's greeting, but the called user's greeting and the recorded message would go into the secretary's vm.
Thanks for all your help, I think I'm going to go with the alternate greeting way, even if they hear the wrong greeting. Otherwise, if I don't play the greeting for the secretary with a call handler, it doesn't if someone calls her directly, we don't want that either.

For the call flow that you are describing to work, you would need to
change the parameter "Use Last (Rather than First) Redirecting Number
for Routing Incoming Call" under System Settings -> Advanced ->
Conversations.
Just to confirm, you want the user to be able to control whether all
calls to them go to his/her own voicemail box or their secretary's? An
easier way might be to use the alternate greeting. For the boss, on
the alternate greeting set the "Callers Hear" to "Nothing", then set
the after greeting to send to "user with mailbox" and pick the
secretary. Then, the boss can just dial into his/her mailbox to enable
it. You don't even have to listen to the prompts if you memorize the
menu numbers. I just tried with a phone here using the standard
conversation, I dial 412 then hang up. The next time you dial in to
the mailbox, you will be prompted for whether or not you want to turn
the alternate greeting off. You can also toggle the alternate greeting
from PCA, if preferred.
If they just need to do this after hours, you could do the same thing
with a schedule, using the closed greeting.
