Ah! That's it. At least I can do with without route points in CM, and
I could do it all with call handlers (no alternate contact number) to
lend it more consistency....

You might check your restriction tables re: the outbound to the outside
number.

I've done what you suggested. I can't set the Standard Transfer Rule to
an outside number, or I can, but the call doesn't go. I can set it to
an internal number on a route point and from there forward the call to
the outside world. Should I be able to put an outside number in that
Transfer field?
And it's really true that on a single subscriber, I can't use multiple
alternate contact numbers but can only assign the same one to multiple
input keys? Seems weirdly limited to me.
So my setup will be:
Press 1 to go directly to a subscriber's VM (set to alternate contact
number)
Press 2 to go directly to another subscriber's VM (set to a call handler
which transfers to another subscriber's VM)
Press 3 to go to a route point in CM (set to a call handler which
transfers to a route point which forwards to outside number)
Press 4 to go to a route point In CM (set to a call handler which
transfers to a route point which forwards to outside number)
And if no input is given, caller is sent to VM of the original called
number.
This will work, but it seems like a lot of running around to do
something pretty simple sounding--or am I still missing something?

Questions
For option 3, you need to create a call handler. That call handler will
have a Standard Transfer rule to the outside number. Same for option 4
(different call handler). These CHs do not need an extension. Just
associate them to those options.
Be aware that in Unity 5, I believe the Alternate transfer rule is
enabled by default, so be sure to disable it (bug I think).

I want to use Unity to give a caller who reaches VM to have 4 options:
Press 1 to go directly to a subscriber's VM
Press 2 to go directly to another subscriber's VM

And if no input is given, caller is sent to VM of the original called

In looking at the Subscriber's caller input choices or in a Call
Handler's caller input choices, I'm not seeing how to do this.
In the Subscriber's caller input, apparently I can only set a single
Alternate Contact Number. While I can set input numbers 1, 2, 3, to
send to an Alternate Contact Number, that number is always the same for
all 3 input numbers. If I can change one Alternate, they all change.
In a Call Handler, I can't see where I can get a call back to call
manager to send it out into the world.
This can be done, surely. Any suggestions to help me with what must be
bleedingly obvious....
