So what URL is in your service definition? The document you specified clearly states that the default URL is IPAgentInitial.jsp and the one-button login needs to call IPAgentLogin.jsp. If you are giving the parameters to IPAgentInitial.jsp it won't care, and will behave exactly as you are seeing.

Make sure you are calling the correct URL in the service definition. A packet capture will show you if the destination URL for submitting the login is the same as the one used for the initial request.

I have used this with UCCX 4 up to 7 so far with no problems.
Make sure the ID, Ext, Pwd variables are exactly how they appear in
the doc, I think they might be case sensitive.Might just have one of
them off and so it re-prompts for the login.
If you want to really make it easy for them and have enough buttons,
you can also setup service buttons for "ready" etc as well. they are
just calls to URL with different parameters.
