Bingo. Adding Notify fixed it.

What did you set for access privileges did you set on the community string?  I didn't see one until I created one as  "Notify Only".  I assume a trap destination requires a notify enabled string.

SNMP Notification Destination Configuration

Community String <Drop Down>  is empty

In Tools > Control Center - Features Services I've restarted Cisco CallManager SNMP Service but still don't get anything under Notification Community String drop down, even though SNMP > v1/v2>  Community String shows the community string I entered. I even tried delete community string/restart SNMP/add string with the Create New Community String in the Notification Destination menu.
