[cisco-voip] Upgrade 5.1.3 to 7.1.3

I'm having a problem at a customer upgrading 5.1.3 - 7.1.3. Done it twice already with a brand new 5.1.3-7.1.3 upgrade DVD. Logged TAC case, but not having much luck.

After going through the logs, I found the following. Any ideas?
