[cisco-voip] FXO ports and registering with CCM

The common problem is forgetting to add the domain name to the name
entered in CUCM.

Do a 'show mgcp' and it should say the name it's registering with (2nd
line I believe).  It will be hostname.domain.com

The router never knows whether or not a FXO or FXS port is plugged in,
so it should register regardless.

If you do a 'show voice port summary' you should see whether the ports
are in a good state or not.

'debug mgcp packet' is more helpful for determining the cause.

Sometimes resetting the TFTP service in CCM helps with the TFTP errors.
