[cisco-voip] IP Phone sets TFTP server to 255.255.255.255

What type of phone, what version of Firmware.  There was a firmware bug that
would do this for the 7041/61 Phones.  Unfortunately the solution was to
hardcode the tftp server to upgrade the firmware.
