Try to reset user password and make the password to never expire and
don't prompt the user to change the password at next login "strange I
know but I faced a similar case"

When this user's DN forwards to Unity (5.0), the caller hears the Unity
Opening Greeting. The user is set up with a subscriber in Unity with a
matching DN. The Voicemail Profile is the same as the one being used by
everyone else.

I am thinking that the line that has "strSubscriberID = " is
significant, because it is populated with the DN/subscriber ID in other
successful forwards to Unity.
I even modified the standard VM Profile and assigned to this user's DN
with a mask containing his DN.
I can't see any differences in either the phone/DN setup or the Unity
subscriber setup.
Could there be an issue with the user's subscriber record/mailbox?
Can anyone offer any hints? I am banging my head.
